By submitting this application, I attest (check box) that the above information is true and correct to the best of my knowledge, and that I meet the pre-requisites for this training course. I hereby give permission to the Chartered Training Program to verify any of the above information and I will provide copies of all certifications at the time of application. I understand that I will be required to sign my application upon acceptance to this course.

Upon receipt of a confirmation for this course, I also understand that I will be responsible for 100% of all course tuition and fees unless I notify the program in writing of my intent to withdraw no later than 48 hours prior to the first class date.


 

    PRE-REQUIREMENTS FOR ADVANCED COURSES:

In addition to the above requirements, the student meets the following admission requirements:

Pre-requisites
Volunteer Firefighter Completion of Hazmat Awareness/Operations (or concurrent with course).
Firefighter 1 Transition Completion of Volunteer Firefighter course (submit copy of card)
Firefighter 1 None
Firefighter 2 Transition Completion of Firefighter 1 course (submit copy of card)
Firefighter 2 None
Fire Safety Inspector 1. Must be employed by one of following:
    a. Office of State Fire Marshal;
    b. A firefighting agency;
    c. Private fire company that provides fire protection.
2. Submit a letter for recommendation from employer.
3. Must hold a current firefighter certificate (submit copy of card)
Fire Instructor 1. Minimum of 5 years work experience as Firefighter or Fire Safety Inspector, including 3 years immediately prior to course;
2. Successful completion of knowledge exam (80%+).
ICS - 300 Completion of ICS-100 and ICS-200
ICS - 400 Completion of ICS-100, 200 and 300
